Job Purpose

To act as an advanced autonomous practitioner, demonstrating a high level of practice and knowledge whilst participating in direct evidenced based patient care. You will be part of a multi-disciplinary team providing professional and clinical support in the care of patients.

To undertake autonomous health assessments; diagnosing, treating and discharging patients within own competence.

The Advanced Clinical Practitioner (ACP) will provide expert care and treatment to patients within agreed protocols and clinical guidelines. However, the ACP will also have the ability to make decisions outside these frameworks, where necessary based on best practice.

Provide support within the team and act as role model and resource, encouraging and developing a high standard of clinical skill and improving patient outcome.

Functions in a variety of role dimensions: health care provider, educator, researcher, clinical advocate and implementer of clinical governance.

With clinical colleagues lead the development of clinical services within community services and improve the quality of care delivered.

Duties and Responsibilities

To work within the GP Ooh service working in base, home visiting and telephone triage managing acute illness presentations.

Act as a clinical expert taking a lead role in advancing the development of clinical practice.

The post holder will provide holistic patient care within a variety of settings, inclusive of the patient’s residence and care settings within the community, from assessment of presenting condition through to diagnosis, initial treatment and discharge, including the administration of medication within agreed clinical guidelines and protocols.

Be aware of meeting national targets and work with the directorate and stakeholders to achieve these.

Act as a resource for the non-medical team within the community directorate, the trust and the wider health community as required.

To act in a professional manner at all times. in accordance with professional body.

To actively engage in the teaching, mentorship and supervision of junior staff and students.

To contribute to the organisational management of the service on issues such as health and safety, use of supplies and following local policy regarding incident reporting.

The post holder will be expected to demonstrate a significant commitment to providing a quality, evidence-based service, excellence in clinical practice, this together with continuous professional development are integral to the role and development of the service.

To work in accordance with the NMC and HCPC rules of professional conduct and within local care pathways, policies and standards.

Be professionally and legally responsible and accountable for all aspects of his/her own professional activities and to manage clinical risk within own caseload at all times, whilst remaining responsible to the Clinical Lead for their Service.

Manage resources within the health economy, ensuring effective use is made of physical and financial resources and make recommendations where appropriate.
