About the Role

We are currently seeking an experienced Band 7 Specialist Pharmacist to join the Older Persons Medicine team at Portsmouth Hospitals University NHS Trust on a locum basis.

This is an excellent opportunity for a pharmacist with strong clinical experience who is looking to work within a specialist older persons medicine setting and contribute to high-quality, patient-centred pharmaceutical care.

Key Responsibilities

  • Provide specialist clinical pharmacy services to patients within Older Persons Medicine.
  • Work closely with the multidisciplinary team to optimise medicines and improve patient outcomes.
  • Undertake medicines reconciliation, clinical medication reviews and pharmaceutical care planning.
  • Provide specialist pharmaceutical advice to medical, nursing and other healthcare professionals.
  • Promote the safe, effective and cost-effective use of medicines.
  • Identify and manage medication-related risks and contribute to medicines governance.
  • Support appropriate prescribing and medicines optimisation in older and often complex patients.
  • Contribute to the development and improvement of clinical pharmacy services.
  • Provide education and training to healthcare professionals and pharmacy colleagues where required.
  • Work in accordance with Trust policies, procedures and relevant medicines legislation.

Essential Requirements

  • GPhC registration as a Pharmacist.
  • Previous NHS hospital pharmacy experience.
  • Strong clinical pharmacy experience at Band 7 or equivalent level.
  • Experience working within a multidisciplinary team.
  • Good knowledge of medicines optimisation and clinical governance.
  • Excellent communication and organisational skills.
  • Ability to work independently and manage a varied clinical workload.

Desirable Experience

  • Previous experience in Older Persons Medicine / Geriatrics.
  • Experience managing complex medication regimes and polypharmacy.
  • Experience of medicines optimisation in elderly or frail patients.
  • Experience providing education and training to healthcare professionals.
