Mediplacements are currently recruiting for a GP Practice based Pharmacist for a Locum job in the West Midlands*Up to £35

Looking for a pharmacist with medicines optimisation experience in general practice with experience using SystmOne
Further details as follows:

1.To contribute to discharge advice note reconciliation, medication reviews/reauthorisations
2.Provide professional specialist medicines management advice to GPs, practice nurses, community pharmacists, community nurses and other health professionals to promote the safe, effective, and high quality, cost effective use of medicines in all prescribing decisions.
3.To identify, develop and promote quality improvements in medicines management across MMP ensuring that best practice is adopted
4.To carry out medication reviews of patients on repeat medicines and amend prescriptions according to patient needs or changing conditions
5.To add / amend medication in patient records from hospital letters
6.To work with the CCG's medicines management team to agree target areas for MMP and amend medication accordingly in consultation with the patient.

This role is not Patient facing,

In order to qualify for this position, it is essential that you hold an MPharms Degree, as well as a registration to GPhC. Recent GP Practice based experience is essential to be consider for this position.
