The government is set to introduce changes to the NHS to make it better able to deal with the forecasted increase of older people with long term illness.
It is believed that the incidence of people with long term conditions will increase by 252 per cent by 2050, which health secretary Andrew Lansley said the NHS will not be able to cope with unless changes are made.
He announced that the new NHS would give people with long term illnesses more support to be able to care for themselves and use new technology which means they can stay independent for longer.
Additionally, the measures will simplify which doctor cares for each patient and ensure they look after their whole wellbeing rather than one specific condition.
"Patients don't want to see lots of different people for lots of different consultations – they want one team who can give them the advice, support and care they need," Mr Lansley said.
"The NHS in the future will give patients better and more efficient care, keeping people well and out of hospital."
